EMPLOYEE HEALTH DATA MANAGEMENT
The results of any health promotion program lose significance if they are not measured, analyzed, and communicated to payers with strong ROI. Our data management expertise helps organizations document success of prevention and wellness initiatives and can assist with health outcomes measurements. Our comprehensive software applications work with your existing vendor programs to generate HIPAA-compliant, de-identified reports to identify trends and predict outcomes for participant groups. Individual, personalized predictive models can also inform participants of their chronic health risks and provide insight into the modifiable behaviors that can be altered to reduce their risk.
Health Promotion Solutions has a portfolio of unique services and tools to support effective data management and health outcomes.
Data Warehousing and Aggregate Reporting
We gather and compile meaningful biometric, personal health and medical claims information to be stored in our HIPAA-compliant data warehouse. All information is de-identified and stored on secure, encrypted servers. We are able to analyze this data to generate customized reports to provide actionable and measurable outcomes.
We utilize proprietary software to create HIPAA-compliant reporting to:
- Evaluate a population’s specific disease risk
- Predict and assess a population’s future disease burden and the associated health management costs
- Measure and track ROI
- Build a consumer database
Market Research and Post-Program Employee Health Assessments
Through customized market research surveys and post-program assessments, we can gauge and evaluate participants’ behavior change and program impact. We create client-specific surveys to evaluate program perception, participant attitudes, and follow-up activities.
- Determine program efficacy and participant impact
- Evaluate opinions towards sponsor products
- Assess perceived and tangible advantages to specific products
- Gather information regarding post-program behavior
- Generate longitudinal tracking reports
Through effective marketing research and feedback, we can help determine the success of a program and devise beneficial ways to reach out to participants. By building on the momentum gained by the promotion, follow-up activities can be planned to assess long-term outcomes.
Employee Health Records
An electronic personal health record (PHR) provides a complete and accurate summary of the health and medical history of an individual by compiling data from multiple sources. Maintaining a current, accessible PHR can help improve the quality of care a patient receives by reducing or eliminating duplicate tests and allowing patients to receive faster, safer treatment and care in an emergency. Using state-of-the-art technology to encrypt and store PHR information on our secure, HIPAA-compliant server, our health records system provides access only to authorized individuals.
Predictive Risk Assessment
Driven by a powerful, patented predictive technology, we utilize the first and only multi-dimensional tool to predict the risk of onset and progression related to multiple chronic diseases. It allows individuals, providers and employers to accurately identify, quantify and graphically visualize risk of major chronic diseases. More importantly, the tool reveals how much the risk of onset can be reduced as well as the optimal pathway to achieve that reduction. The program is completely evidence-based, which means personalized health risk assessment at its best.
We use a patented, empirically-based, statistical tool to comprehensively synthesize risk information derived from over one hundred clinical studies into powerful predictive risk assessment models. The underlying methodology enables predictive equations to be constantly updated to reflect the latest research findings regarding chronic disease risk factors, thus making it the most accurate risk assessment tool available today.
Unlike traditional HRA tools, our assessment is clinically-validated and grounded in the latest medical science. Because of this fact, it can be used to bring an often-missing piece of the health puzzle back into the picture – the physician. Evidence shows that a conventional HRA is unable to change patient behavior but effective one-on-one risk communication can and the effect is even stronger when delivered by a physician.
Population Health Analysis
With a focus on patient centric care coordination and Pay for Outcomes (P4O) reimbursement models, our population health analysis compiles data from all areas and providers involved in member health management. Using results from health risk assessments, biometric data, medical, pharmacy and disability claims, we compile data onto a single dashboard, and provide the tools necessary to ensure partners are in appropriate interventions. We are launching our new physician guided care coordination networks – sometimes referred to as clinically integrated or patient centered medical homes – that are gaining momentum among employers and transforming how healthcare and management is delivered and paid. We use our proprietary information infrastructure to integrate other relevant more real time data from the patient’s physician(s) to track patient progress and identify gaps in care, so that our clients achieve optimal financial and healthcare outcomes.
We take quality seriously and provide reliable, easy-to-use technologies that empower employer groups and/or physicians to reap the benefits of focusing on quality of care. Unlike traditional data warehouses, our data-station allows seamless delivery and evaluation even if the employer changes insurance carriers or programs. Our analysis is the central nervous system and integrates seamlessly with other systems across the care continuum. It leverages data analysis and does the legwork required to independently audit performance and quantify impact, ensuring the employer gets desired results, and/or when able to implement the PCC program, putting the physician back in the driver’s seat for coordinating care.
